Agency Selection & Intake Process

On February 18, 2024, I chose to work with the agency side of Surrogacy Is, which opened my agency intake profile portal.


On February 19, 2024, I completed the full intake profile, minus the home video. The intake profile asked for information including:

  • My basic information

  • Match preferences

  • Photos and letter to IPs

  • Insurance information

  • Pregnancy information

  • Birth details

  • Health

  • Documents (IDs, insurance cards and documents, paystubs)

  • Family lifestyle

  • Surrogate information

  • Employment

  • References

  • Home video


On February 19, 2024, I also received a text from the agency intake coordinator to schedule our intake phone call. Our call was scheduled for February 23, 2024.


On February 23, 2024, I had a new patient appointment with a new primary care physician in the morning and the agency intake call in the afternoon. During the intake call, we went through the pages of my intake profile (listed above) to ensure everything was filled in correctly. Then we reviewed next steps. Following the phone call, the agency intake coordinator would send me a few emails for the following:

  • Release of information forms to collect medical records from previous OBGYN offices and hospitals in which I gave birth (I signed the forms the same day 2/23/2024). The offices/hospitals have 30 days to respond either with my records or informing them that they no longer have my records

  • Benefits package forms to sign confirming the compensation package the agency offered me (I signed same day 2/23/2024)

  • OBGYN Clearance form to have printed and signed by my last OBGYN office confirming that I am physically capable of handling another pregnancy (I scheduled an appointment with OBGYN office for 3/7/2024)

  • Informed me to have an updated pap smear and send results to them (I scheduled an appointment for pap for 3/5/2024)


On March 4, 2024, we had a final profile review video call. This was a virtual meeting where we reviewed my profile one last time and discussed next steps. Next steps from this call were to:

  • Complete and sign background check forms (my husband and I signed same day 3/4)

  • Reminder for me to have updated pap smear (pap completed 3/5/2024 and results sent to agency 3/13/2024)

  • Reminder for me to have OBGYN Clearance form signed (was signed and sent to agency on 3/7/2024)

  • Record home video to show all living spaces, rooms, bathrooms, and exterior of home to ensure we live in a safe environment (video recorded and sent to agency on 3/15/2024)


As of March 15, 2024, all of my steps were completed. The next step was to wait for all medical records to be sent in and reviewed (as of 3/22/2024, the only remaining records not sent yet were 2015 and 2017 OBGYN office records, everything else had been sent to the agency).


Once the remaining records are sent in, the agency will:

  • Review records and comfirm I had uncomplicated pregnancies

  • Run my husband and my background check

  • Review my home video recording and other documents I sent in


When these steps are complete amd were cleared toove forward, we will move into the matching phase.

EXPLANATION OF SURROGACY IS: Surrogacy Is originally started as a advocate agency. They would match potential surrogates to agencies across the US. These agencies were vetted by Surrogacy Is as ethical and legit agencies. Within the last few years ago, Surrogacy Is opened an agency side. Meaning, they are still an advocate agency, but they also have an agency side. Agencies work directly with surrogates and intended parents (IPs) to support both parties through the matching and surrogacy process.
